Use this form to request reimbursement of 60% of Internet costs (up to $35 per month) in order to access the internet for home-learning purposes at your child(ren)'s primary home address.
To qualify for the full amount in January for the fall term (covering Sept, Oct, Nov, Dec, and Jan) HOHL must receive a copy of your September bill for internet by December 15th at the latest.
To qualify for the full amount in June for the spring term (covering Feb, Mar, Apr, May, and Jun) HOHL must receive a copy of my February bill for internet by May 15th at the latest.
If your Internet Service Provider changes, or there is any change to the amount of your bill, you must also notify HOHL and submit a copy of your bill for the month of the change.
Reimbursement cheques will be mailed out at the end of January (for September to January) and the end of June (for February to June) in the name of the bill payee.
You must be active in the program to receive an internet reimbursement. This means you must be communicating regularly with your teacher and be up to date on weekly work sample and reflection question submissions for each month that you are wanting reimbursement.
